How to Register
To register with the practice you will need to contact
the surgery and inform us that you wish to register. You
will be eligible to register if you live within our
practice area and are not
already registered with another surgery in the area.
When you register you will need to provide 2 forms of
identification chosen from the following list:

National Ins Card
Birth Certificate |

Paid utility bill/Official
change of address
Local Authority rent card
Bank card/statement
Wage slip |
Please choose one from each list.
After completing the relevant registration forms you
will be asked to make an appointment with the Practice
Nurse so that information can be obtained about your
past and present health problems and treatment.
Patients register with the Practice, not with an
individual GP. When making appointments you may express
a preference as to which doctor you would like to see.
However, if you request a particular Doctor please be
aware there may be a wait to see him/her.
