Care Coordinator –

I have recently joined the practice having worked in health and social care for the last 19 years. I have vast experience in the community as well as managing residential homes for learning disabilities, dementia and mental health, which are my specialist areas. I have an NVQ level 5 in Health and Social care, an HND in forensic psychology and am currently finishing an open degree. As well as work and studying I am a mother and am dedicated to helping those around me in making their world a better place.

My role within the surgery is to signpost and empower patients to engage in community services with the aim of reducing GP visits and avoiding hospital admissions. I am an advocate for patients and their families and can liaise with outside agencies/charities/services on their behalf to provide a person centered approach to the care and over all well-being of our patients. I can provide information and advice on all aspects of our patients social needs and a safe and friendly environment to talk in. I am available from Monday to Friday, 9am till 2pm on 01444 458738 and can see patients at either Cuckfield or The Vale.
